Code of Conduct, Safeguarding, Accident and Incident Referral Forms

Player Code of Conduct Referral Form
The 'Player Code of Conduct' form is for use of any incidents relating to or violating to a players agreement to the club code of conduct, relating to Saint Francis Hockey Club. Any incidents should be reported using this form and will be reviewed by the relevant committee members of the club.

Safeguarding Referral Form
This form is to be used to notify Saint Francis Hockey Club of any suspected or actual instances of abuse or neglect to anu individual at the club. Please complete form for any concerns.

Accident Form
The incident in question has resulted in physical harm to a member of the club, or member of the public. Report any incidents that required first aid; except for 'bumps and scrapes.

Minor 'Bumps and Scrapes' Form
The 'Bumps and Scrapes' form is for use of any minor injuries when playing or around SFHC facilities, where there no injury apparent, or the injury is minor in nature (requiring basic first aid). It is less detailed than the accident form, and is quick for a small record of incidents.
